I think the main reasons for the decline is he has been injury ravaged for the past 3 or so seasons.

JO has had a string of injuries over the last 4 seasons, most of which have been either unlucky (he has had hyperextended limbs a couple of times) or the result of having to bulk up too much to play Center.
He was an MVP candidate the year that the Pacers had the best record in the NBA.
Oh and I'm sure there are bigger foreheads in the NBA.

I think you should all check out Aaron Rice Williams AKA Ricey.
That dude has a fore head and face that could stop traffic.

WOW, Jermain has a bigass forehead! People look so different without headbands, LeBron head looks pretty big (literally, I mean) without his headband. I've heard many people say that Indiana desperatly need to trade O'neal, getting worse every year and big contract.
